Once I save my form how long until the link expires.

You have 7 days before the saved liked will expire.

Once I delete my form submission is it deleted forever?

Yes, we do not store any client/patient information on our servers once you have deleted it out of your account.

Is there a limit to how many forms I can have?

Yes and No. If you are on our unlimited plan you can have as many forms as you need, however, if you are on the starter plan there is a limit of 5 forms.

Does FormDoctor have a referral program?

Yes. Login to your account and go to My Account > Referral. You will receive a $50 referral credit as well as any person you refer.

Are my phones mobile friendly?

Yes. Our forms will shrink down to the appropriate size whether they are using a computer, tablet or phone.

Can I filter my submissions?

Yes. You can filter submissions by Completed, Incompleted, Opened, Received but not open, Sent by not received, and by the form name.

Can we view the forms sent by most recent submission?

Yes. To filter your submissions by most recent, go to “Filter Submissions” and filter by “Completed.” By default, this will load the most recent submission.

When a client fills out partial forms at home and they come in to finish forms, how do they finish where they left off?

If a client started to fill out a form and selected “Save and Continue Later” they will have a secure link emailed to them. They can gain access to that form from their email using the last four digits of their social security.

If you delete the client intake file, do they go away forever?

Yes, we do not store client data after it has been deleted.

What happens when I select “Save Progress”?

Once you select “Save and Continue Later” you will be asked to enter your email address and the last four digits of your Social Security. Once you have entered your social and email a secure link will be emailed to you. You will need the last four digits of your Social Security to access this information.

Can I view a form that says “Saved Progress”?

No, while the form is in “Save Progress” mode only the client will have access. Once the client has completed the form you will be able to access the completed information.

Does FormDoctor provide a BAA?

Yes, we offer a Business Associate Agreement (BAA).

Is FormDoctor HIPAA compliant?

Yes, we store our information on the same secured servers as major hospitals systems.

 

Are we able to modify the form ourselves?

Yes. Go to “My Forms > Revisions” to make updates to your form.

Is there a limit to the amount of revisions I can make?

No, you have full access to your form and can make as many changes as you need.

Can we customize our form?

Yes, FormDoctor has a FormBuilder that allows you to edit the logo and colors. You can also adjust the questions to fit your business needs.

When I enter a cell phone number and email, does the form go to both places?

Yes, if you enter the email and phone number they will receive a text message and an email asking them to fill out the form.

Does FormDoctor translate forms to a different language?

We currently do not offer any translation services. However we can run a form through Google Translate, but we can not guarantee the accuracy of the translation.
